Overview

Germ cell tumors (GCTs) are a heterogeneous group of neoplasms that arise from primordial germ cells, the precursor cells that normally develop into eggs or sperm. These tumors can occur in the gonads (ovaries or testes) or in extragonadal sites along the midline of the body, including the mediastinum, retroperitoneum, sacrococcygeal region, and central nervous system (particularly the pineal gland and suprasellar region). GCTs encompass several histological subtypes, including germinomas (seminomas/dysgerminomas), yolk sac tumors (endodermal sinus tumors), embryonal carcinomas, choriocarcinomas, teratomas (mature and immature), and mixed germ cell tumors. They can be benign or malignant. Clinical presentation varies depending on the tumor location and subtype. Gonadal GCTs may present as a painless testicular mass in males or an ovarian mass causing abdominal pain, distension, or menstrual irregularities in females. Extragonadal tumors may cause symptoms related to mass effect, such as respiratory distress (mediastinal tumors), neurological symptoms including diabetes insipidus and visual disturbances (intracranial tumors), or bowel and bladder dysfunction (sacrococcygeal tumors). Many GCTs produce tumor markers, including alpha-fetoprotein (AFP) and beta-human chorionic gonadotropin (beta-hCG), which are valuable for diagnosis and monitoring treatment response. Treatment depends on tumor type, location, and stage, and typically involves a combination of surgical resection and platinum-based chemotherapy (most commonly bleomycin, etoposide, and cisplatin — the BEP regimen). Radiation therapy may be used for certain subtypes, particularly intracranial germinomas, which are highly radiosensitive. Overall, GCTs are among the most curable solid tumors, with cure rates exceeding 90% for many subtypes when treated appropriately. Risk factors include cryptorchidism (undescended testes), disorders of sex development, and certain genetic conditions such as Klinefelter syndrome. While most cases are sporadic, familial clustering has been observed, particularly in testicular GCTs, suggesting a multifactorial genetic contribution.
