Overview

Diffuse lymphatic malformation is a rare vascular anomaly characterized by widespread, abnormal development of lymphatic vessels that affects multiple body regions or an entire anatomical area rather than forming a localized mass. Unlike focal lymphatic malformations (previously called lymphangiomas or cystic hygromas), the diffuse form infiltrates tissues extensively, making it particularly challenging to manage. The malformation is composed of abnormally dilated lymphatic channels that can involve the skin, subcutaneous tissues, muscles, bones, and internal organs. Key symptoms depend on the anatomical regions involved but commonly include progressive soft tissue swelling, skin changes such as lymphatic vesicles that may leak lymphatic fluid, recurrent infections (cellulitis or lymphangitis), pain, and functional impairment of affected structures. When involving the thorax or abdomen, patients may develop chylous effusions (pleural or peritoneal). Skeletal involvement can lead to bone overgrowth or destruction. Coagulopathy, including localized intravascular coagulopathy, may also occur in extensive disease. Treatment is multidisciplinary and often palliative rather than curative due to the diffuse nature of the condition. Management options include compression therapy, sclerotherapy, surgical debulking (though complete resection is rarely possible), and medical therapies such as sirolimus (an mTOR inhibitor), which has shown promise in reducing the size and symptoms of lymphatic malformations. Supportive care includes management of infections, pain control, and treatment of complications such as chylous effusions. The condition is typically present from birth or early childhood and tends to be progressive without intervention.
