Overview

Rare female infertility due to an adrenal disorder is a condition in which problems with the adrenal glands — small organs that sit on top of the kidneys — lead to difficulty becoming pregnant or the inability to conceive. The adrenal glands produce important hormones, including cortisol (the stress hormone), aldosterone (which helps control blood pressure and salt balance), and androgens (sex hormones). When the adrenal glands do not work properly, the balance of these hormones can be disrupted, which may interfere with ovulation, menstrual cycles, and overall reproductive function in women. The most well-known adrenal disorder linked to female infertility is congenital adrenal hyperplasia (CAH), particularly the non-classic or late-onset form, where excess androgen production can suppress normal ovarian function. Other rare adrenal conditions — including adrenal insufficiency, adrenal tumors, or enzyme deficiencies in the steroid hormone production pathway — can also contribute. Symptoms may include irregular or absent periods, excess body or facial hair, acne, weight changes, fatigue, and difficulty conceiving. Treatment depends on the specific underlying adrenal disorder. Hormone replacement therapy, glucocorticoid medications (such as hydrocortisone or dexamethasone), and fertility treatments may be used. With proper diagnosis and management, many women with adrenal-related infertility can achieve successful pregnancies. Early recognition and referral to both an endocrinologist and a reproductive specialist are important for the best outcomes.
