Overview
Neuropathy with hearing impairment is a rare hereditary condition characterized by the combination of peripheral neuropathy and sensorineural hearing loss. This disorder affects both the peripheral nervous system and the auditory system. Patients typically experience progressive weakness and sensory loss in the extremities due to peripheral nerve damage, alongside progressive hearing impairment caused by dysfunction of the auditory nerve or cochlear structures. The peripheral neuropathy may manifest as numbness, tingling, pain, and muscle weakness, predominantly in the distal limbs, with reduced or absent deep tendon reflexes. Electrophysiological studies typically demonstrate features consistent with a demyelinating or axonal neuropathy. Several genetic forms of hereditary neuropathy with hearing impairment have been described, and the condition may overlap with certain subtypes of Charcot-Marie-Tooth disease (such as those caused by mutations in genes like MPZ or GJB1) or other syndromic neuropathies. The age of onset and severity can vary depending on the underlying genetic cause. Currently, there is no curative treatment for this condition. Management is supportive and multidisciplinary, involving neurologists and audiologists. Treatment strategies include physical therapy and rehabilitation to maintain mobility and function, orthotic devices for foot drop or gait abnormalities, pain management, and hearing aids or cochlear implants to address hearing loss. Genetic counseling is recommended for affected individuals and their families.
